CMS Selects New Sites for Care Transitions Program

The Centers for Medicare & Medicaid Services (CMS) announced 17 new sites that will participate in the Community-Based Care Transitions Program (CCTP), bringing the total number of sites to 47.

The CCTP was established by the Affordable Care Act to test models for improving care transitions for Medicare recipients — and therefore reducing hospital readmission rates and saving Medicare money.

Currently, one in five Medicare recipients is readmitted to the hospital after being discharged, at an estimated yearly cost of $26 billion.
