One Vision: Moving Forward — Michigan removes barriers to person-centered practices in nursing homes

Provides an in-depth look at the One Vision: Moving Forward initiative, a multi-year project in Michigan designed to advance person-centered practices in the state’s nursing homes. PHI Michigan convened meetings of key long-term care stakeholders, who worked collaboratively to to identify the barriers impeding the implementation of person-centered practices, and then remove those barriers. Using a consensus-based decision-making process, the stakeholders and PHI produced a series of “clarification documents,” which named real or perceived barriers to person-centeredness, and then outlined the steps that can be taken to ensure full person-centeredness in Michigan’s nursing homes.
