Center to Advance Palliative Care

New York, NY

The Center to Advance Palliative Care (CAPC) is the nation’s leading resource for the development and promotion of palliative care. As a national, member-based organization, they provide hospitals, health systems, hospices, payers, and other health care organizations with the tools, training, technical assistance, and metrics needed to support the successful implementation and integration of palliative care.

Most recently, PHI partnered with CAPC to improve the quality of care for about 1,000 older adults and people with disabilities through the integration of a palliative care approach to home care. Together, we are piloting new curricula and program elements on palliative care with a New York City-based network of home care providers and managed long-term care plans. Our goal is to improve how managed care plans and home care workers adopt this new palliative approach, as well as to improve the quality of palliative home care services for people living with serious illness. To achieve this goal, we deliver trainings that build the skills of direct care workers in delivering palliative care models, which are often absent in core training curricula.

As a provider of the highest-quality training content for direct care workers, PHI is a natural partner for the Center to Advance Palliative Care. Through our collaboration, we hope to embed a palliative approach in the care this critical workforce provides to our country's vulnerable, seriously ill patients.
